Partner Referrals

Neighbors Light is a referral-based program. We will work with trusted partner agencies and case managers to ensure we can provide the right level of care and support for each individual.

Referrals will open once programs begin accepting participants.

How Referrals Work

Neighbors Light is a referral-based program. We do not accept walk-ins. All participants will be referred by trusted partner agencies, case managers, or service providers who know the individual's needs and can confirm that our services are a good fit.

This referral process will ensure that we can provide the right level of care and support, and that participants are prepared for the structure and expectations of our programs.

Who Can Submit a Referral

Referrals may be submitted by:

  • Case managers at emergency shelters or outreach programs
  • Social workers at hospitals or medical facilities
  • Staff at mental health or substance use treatment centers
  • Veterans' services coordinators
  • Probation or parole officers
  • Faith-based organizations with established case management programs

If you are an individual seeking services, please contact one of these organizations in your community. They will be able to assess your needs and submit a referral on your behalf.
